Tomeca Nicole Hooper :

Tomeca Nicole Hooper: Tomeca Nicole Hooper secured employment and practiced as a Licensed Vocational Nurse (LVN) with a Home Health Care agency in Dallas, Texas, from March 17, 2025, through June 23, 2025, by using the license number, social security number, date of birth and presented other forms of identification belonging to another nurse with a different name (“Lanette Payton”). During this time, Tomeca Nicole Hooper provided direct patient care to pediatric patients and represented herself as a Licensed Vocational Nurse to staff, patients, families, and the public while utilizing the credentials “LVN” on medical records. This case has been referred to the Office of the Dallas County Attorney’s office, and Dallas City Prosecutor for prosecution. Previously, Tomeca Nicole Hooper was featured in the October 2024 Bulletin under the alias of “Mindy Land, LVN,” where she secured employment and practiced as a Licensed Vocational Nurse in home health and private nursing in Fort Worth, Texas, from March 2023 through April 2024. |
Sarah Nicole Flores:

Sarah Nicole Flores: Sarah Nicole Flores attempted to secure employment and practice as a Registered Nurse (RN) with a nursing facility in Robstown, Texas, in May 2025, by using the license number belonging to another nurse with a similar name. During this time, Sarah Nicole Flores did not provide direct patient care to patients but represented herself as a Registered Nurse to hiring staff. The Board’s investigation revealed that the date of birth, social security number and identification photo for Sarah Nicole Flores did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. Additionally, Sarah Nicole Flores misrepresented herself to the Texas Board of Nursing and attempted to reactivate a delinquent Licensed Vocational Nurse (LVN) license belonging to a Registered Nurse with the same name. Sarah Nicole Flores submitted documents to the Board in an attempt to gain access to the licensure account belonging to the other nurse and attempted to change the information in the nursing portal to her personal information; to include middle name, address, phone number, graduation date, email and licensure history. This case has been referred to the Nueces County District Attorney’s office, and Office of the Attorney General-El Paso, for prosecution. |
Coti Salazar :

Coti Salazar : Coti Salazar secured employment and practice as a Registered Nurse (RN) with a home health company in Fort Worth, Texas, from January 28, 2025, through September 29, 2025, by using the license number belonging to another nurse with a different name and falsifying a license verification search form. During this time, Coti Salazar did provide direct patient care to patients and represented herself as a Registered Nurse to hiring staff. Additionally, Coti Salazar was terminated from another home health company in Forth Worth, Texas, as a Licensed Vocational Nurse (LVN) for similar behavior. Coti Salzar allegedly submitted fraudulent documentation to gain employment with and provided direct patient care as a Licensed Vocational Nurse from September 6, 2023, through November 11, 2024. The Board’s investigation revealed that the date of birth, social security number and identification photo for Coti Salazar did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. This case has been referred to the Tarrant County District Attorney’s Office, and Office of the Attorney General-Dallas/Fort Worth, for prosecution. |

Alice Aubrey Williams: From February 12, 2025, through June 23, 2025, Alice Aubrey Williams continued to practice as a Registered Nurse (RN) with a nursing agency in Houston, Texas, by failing to notify her employer of the revocation order by the Texas Board of Nursing issued February 11,2025. During this time, Alice Aubrey Williams provided direct patient care to the agency’s patients and represented herself as a Registered Nurse to staff, patients, families and the public while using the credentials “RN” on medical and business records. This case has been referred to the Office of Attorney General, Houston, Texas, and Harris County District Attorney for prosecution. |
Gavvyn M. Grant:
a.k.a. Gavyn Grant, Gavynn Michael Grant, Gavynn Michelle Grant, Nurse Ricochet, Nurse Rico, and Gavin Grant |
Gavvyn M. Grant: Gavvyn Grant secured employment as a contractor and practiced as a Registered Nurse (RN) with children’s camp in La Grange, Texas, in July of 2025, by providing fraudulent and fictitious credentials. During this time, Gavvyn Grant did provide direct patient care to patients and represented himself as a nurse to juvenile campers, parents, staff, and the employer. This individual was also identified as having obtained nursing contract positions in numerous other states through the same camp including Oklahoma, Wisconsin, Indiana, New York, Idaho, Florida, Colorado and West Virginia from 2023 through 2025. This case has been referred to the Office of the Attorney General. |
