Dazael Gloria: Dazael Gloria secured employment and practiced as a Registered Nurse (RN) with Home Health Care Service in Katy, Texas, from August 30, 2022, through October 14, 2022, by using the license number, social security number, date of birth and presented other forms of identification belonging to another nurse with a different first name but same last name. During this time, Dazael Gloria provided direct patient care to pediatric patients and represented himself as a Registered Nurse to staff, patients, families and the public while using the credentials “RN” on medical records. The Board’s investigation revealed that the date of birth, social security number and identification photo for Dazael Gloria did not belong to any nurse having a license or privilege to practice nursing in the State of Texas.
Additionally, Dazael Gloria secured employment and practiced as a Registered Nurse (RN) with a Home Health Care Service in Houston, Texas, from August 24, 2023, through October 23, 2023, by using the license number, social security number, date of birth and other forms of identification belonging to another nurse with a different first name but same last name. During this time, Dazael Gloria provided direct patient care to pediatric patients and represented himself as a Registered Nurse to staff, patients, families and the public while using the credentials “RN” on medical and business records. The Board’s investigation revealed that the date of birth, social security number and identification photo for Dazael Gloria did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. Dazael Gloria previously held a license to practice vocational nursing in the State of Texas, which was surrendered on February 14, 2020. This case has been referred to the Office of the Harris County Attorney’s office, Montgomery County Attorney’s office and Hockley County Attorney’s office for prosecution. |

D’HAVELYN SHARNET HUSTON: D’Havelyn Sharnet Huston remained employed and practiced as a Licensed Vocational Nurse (LVN) with a home health agency in Dallas County and Ellis County, from January 20, 2023, through January 25, 2024, after her license to practice vocational nursing in the State of Texas had been revoked on January 19, 2023. During this time, D’Havelyn Sharnet Huston provided direct patient care and represented herself as a Licensed Vocational Nurse to staff, patients, families and the general public while using the credentials “LVN” on medical and business records. This case has been referred to the County Attorney’s office in Dallas County and Ellis County for prosecution. |

KATHRYN RENE JOHNSON: On or about November 11, 2021, through January 16, 2022, while employed as a licensed vocational nurse with a staffing agency in Brady, Texas, and contracted with a nursing home in Fredericksburg, Texas, Kathryn Rene Johnson was practicing without a license. On or about November 9, 2021, Kathryn Rene Johnson’s license was revoked by the Texas Board of Nursing, Austin, Texas. While employed, Kathryn Rene Johnson was providing direct patient care along with administering medications and other responsibilities designated for a licensed vocational nurse. The Board’s investigation revealed that the date of birth and social security number provided by Kathryn Rene Johnson to her employer does not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Gillespie County District Attorney’s office for prosecution. |

JESSICA RENEE JUDD: Jessica Renee Judd secured employment and practiced as a registered nurse (RN) at a hospital in Wichita Falls, Texas, from June 1, 2020, through January 26, 2021, by using the license number belonging to another nurse with the same first and last name. During this time, Jessica Renee Judd provided skilled nursing care to patients and represented herself as a registered nurse to staff, patients, families, physicians, and the public while using the credentials of “RN” on medical and business records. Additionally, Jessica Renee Judd secured employment and practiced as a registered Nurse at a treatment center in Weatherford, Texas, from June 22, 2022, through April 29, 2023, by using the license number belonging to another nurse with the same first and last name. Furthermore, Jessica Renee Judd secured employment and practiced as a registered nurse supervisor at a clinic in Decatur, Texas, from April 14, 2023, through April 21, 2023, by using the license number belonging to another nurse with the same first and last name. In addition, Jessica Renee Judd secured employment and practiced as the director of nursing at a nursing home in Jacksboro, Texas, from May 3, 2023, through May 11, 2023, by using the license number belonging to another nurse with the same first and last name. The Board’s investigation revealed that the date of birth and social security number provided by Jessica Renee Judd to her multiple employers did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Offices of the Wichita County District Attorney, Wise County District Attorney, Parker County District Attorney, and Jack County District Attorney for prosecution. |

ILEANA LIZETH MENJIVAR LEON: Ileana Lizeth Menjivar Leon secured employment and practiced as a licensed vocational nurse (LVN) at an assisted living facility in New Braunfels, Texas, from September 30, 2022, through April 28, 2023, by using the license number belonging to another nurse with a different first and last name. During this time, Ileana Lizeth Menjivar Leon represented herself as a licensed vocational nurse to staff, patients, families, and the public while using the credentials of “LVN” on medical records. Ileana Lizeth Menjivar Leon administered intramuscular injections, assessed multiple clients, treated wounds, and took verbal orders from physicians. The Board’s investigation revealed that the date of birth and social security number provided by Ileana Lizeth Menjivar Leon to her employer did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Office of the Comal County District Attorney for prosecution. |

SUSANA MARIE SUBIA: Susana Marie Subia secured employment and practiced as a licensed vocational nurse (LVN) with a Rehabilitation Center in Odessa, Texas, from February 8, 2022, through April 13, 2022, and from August 2, 2022, through July 5, 2023, by using the license numbers belonging to two (2) nurses with the same first name but different last names. During her employment, Susana Marie Subia represented herself as a licensed vocational nurse to staff, patients, families, and the public while using the credentials “LVN” on medical and business records. The Board’s investigation revealed that the date of birth and social security number provided by Susana Marie Subia to her employer did not belong to any nurse having a license or privilege to practice nursing in the State of Texas.
Additionally, Susana Marie Subia secured employment and practiced as a licensed vocational nurse (LVN) with a Hospital in Odessa, Texas, from July 18, 2023, through November 7, 2023, by using the license number belonging to another nurse with the same first name but different last name. During this time, Susana Marie Subia represented herself as a licensed vocational nurse to staff, patients, families, and the public while using the credentials “LVN” on medical and business records. The Board’s investigation revealed that the date of birth and social security number provided by Susana Marie Subia to her employer did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. Furthermore, Susana Marie Subia secured employment and practiced as a licensed vocational nurse (LVN) with a Nursing Home in Odessa, Texas, from November 10, 2023, through January 22, 2024, by using the license number belonging to another nurse with the same first name but different last name. During this time, Susana Marie Subia represented herself as a licensed vocational nurse to staff, patients, families, and the public while using the credentials “LVN” on medical and business records. The Board’s investigation revealed that the date of birth and social security number provided by Susana Marie Subia to her employer did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. In addition, Identification photos of Susana Marie Subia did not match photos of actual licensees. This case has been referred to the Office of the Ector County Attorney’s office for prosecution. |
