Millicent Anigwe: On or about March 22, 2021, through July 12, 2021, Millicent Chizoba Anigwe secured a promotion to the position of Graduate Nurse (GN) with a home health care service provider in Dallas, Texas, under the false pretense she was going to sit for the NCLEX and provide nursing school transcripts. During this time, Millicent Chizoba Anigwe was providing direct patient care, signing medical records with the credentials “RN,” and misrepresenting herself as a Graduate Nurse and Registered Nurse. The Board’s investigation revealed that the date of birth and social security number provided by Millicent Chizoba Anigwe to her employer did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. This case has been referred to the Office of the Dallas County District Attorney for prosecution. |
Yoeynis Palmero Gonzalez: a.k.a. Yoeynis P Gonzalez; a.k.a. Yoeynis Gonzalez |
Yoeynis Palmero Gonzalez: On or about July 1, 2022, through September 25, 2022, Yoeynis Palmero Gonzalez secured employment as a Graduate Nurse with an Emergency Center in Houston, Texas, by submitting a transcript from EDP University of Puerto Rico. While employed, Yoeynis Palmero Gonzalez was filling shifts as a Registered Nurse and would be the only nurse on duty to oversee any patients. The Board’s investigation revealed that the date of birth and social security number provided by Yoeynis Palmero Gonzalez to his employer does not belong to any nurse having a license or privilege to practice nursing in the State of Texas. |

Keelan Keon Tae Ke Sean Guidry: Between August 16, 2022, through September 22, 2022, Keelan Keon Tae Ke Sean Guidry secured employment and represented himself as a Licensed Vocational Nurse (LVN) with a staffing agency in Brentwood, Tennessee, and was contracted with a Hospital in Austin, Texas, by submitting a falsified license verification report belonging to a differently named nurse. While employed, Keelan Keon Tae Ke Sean Guidry provided skilled nursing care to residents including, but not limited to, administering medications. The Board’s investigation revealed that the date of birth and social security number provided by Keelan Keon Tae Ke Sean Guidry to his employer did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Travis County District Attorney’s office for prosecution. |

Tiffany Venee Owensby: On or about June 6, 2022, through September 9, 2022, Tiffany Venee Owensby secured employment as a Graduate Nurse at a long-term care facility in Houston, Texas, by submitting a fraudulent diploma from The University of Texas at Arlington, Arlington, Texas. While employed, Owensby provided skilled nursing care to residents and represented herself as a RN to staff and patients while using the credentials of “RN” on medical and business records. The Board’s investigation revealed that the date of birth and social security provided by Tiffany Venee Owensby to the facility did not belong to any nurse having a license/permit or privilege to practice nursing in the State of Texas. The case has been referred to the Harris County District Attorney’s office for prosecution. |

Daniel (Dan) Prasad: On December 12, 2022, the British Columbia College of Nurses and Midwives notified the Texas Board of Nursing that permanent injunctions had been issued on their behalf to enjoin and restrain Daniel (Dan) Prasad from using any of the titles “nurse”, “registered nurse”, licensed graduate nurse”, “nurse practitioner”, “licensed practical nurse”, “psychiatric nurse”, or any related abbreviations to describe his work. Mr. Prasad had presented a fraudulent Texas registered nurse license bearing the name “Daniel Prasad” and date “Jun 20, 2022” (sic). The Texas Board of Nursing has never issued a license to practice professional nursing to Daniel (Dan) Prasad. LIXSEL VEGA: On or about July 27, 2022, Lixsel Sarahy Vega attempted to secure employment as a Licensed Vocational Nurse (LVN) with a hospital in Tyler, Texas, by submitting a falsified licensure record belonging to a differently named nurse. The Board’s investigation revealed that the date of birth and social security number provided by Lixsel Vega to the facility did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. |

Kaitlyn Walters:: Between June 15, 2022, and October 7, 2022, Kaitlyn Faith Walters remained employed and practiced as a Licensed Vocational Nurse (LVN) with a home health agency in Grayson County, Texas, after her license to practice vocational nursing in the State of Texas had been revoked by the Board on June 14, 2022. During this time, Kaitlyn Faith Walters continued to provide direct patient care and represented herself as an LVN to staff, patients, families, and the public while using the credentials of “LVN” on medical and business records. This case has been referred to the Angelina County District Attorney’s Office. |

Misty LaShawn Young: Between May 18, 2022, and July 22, 2022, Misty LaShawn Young obtained employment and represented herself as a Registered Nurse (RN) at two long term care facilities, in Newton, Texas, and DeRidder, Louisiana, by using the RN licensure information of a nurse having the same first and last name. The Board’s investigation revealed that the date of birth and social security number provided by Misty LaShawn Young to the employers does not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Newton County District Attorney’s office for prosecution and to the Louisiana State Board of Nursing. |
