Published in the October 2019 Texas Board of Nursing BulletinNora Nely Avila (Ibarra) Nora Nely Avila (Ibarra) secured employment and practiced as a licensed vocational nurse (LVN) with a home health agency in Laredo, Texas, from July 12, 2017, through August 22, 2018, by using the license number belonging to another nurse with the same name. During this time, Nora Nely Avila represented herself as a LVN to staff, patients, and the public while using the credentials of “LVN” on the facility’s business and medical records. The Board’s investigation revealed that the date of birth, social security number, and address information provided by Nora Nely Avila to the facility did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Office of the Webb County District Attorney for prosecution. |

Published in the April 2019 Texas Board of Nursing BulletinReginald Keith Baisden, Jr Between April 24, 2017, and September 13, 2018, Reginald Keith Baisden, Jr., obtained employment with a social services agency in the Houston, Texas area using falsified licensure information. During this time, Mr. Baisden represented himself as a registered nurse (RN) while using the credentials of “RN” on the agency’s business and medical records. Although the various positions held by Mr. Baisden did not require a nursing license, the agency increased Mr. Baisden’s salary based on the falsified licensure information he provided. Further, on September 26, 2018, Reginald Keith Baisden, Jr., attempted to gain employment as a RN with a medical clinic in the Duluth, Georgia, area, by again submitting falsified licensure information indicating he held a RN license from the State of Texas. Staff at the medical clinic were alerted to the attempt when they tried to verify the licensure information provided by Mr. Baisden and found that the license belonged to an individual with a completely different first and last name. The investigation conducted by the Board revealed that Reginald Keith Baisden, Jr., has never been issued a nursing license and/or privilege to practice nursing in the State of Texas. |

Published in the April 2019 Texas Board of Nursing BulletinDanielle Calderone Gray On or about July 25, 2018, Danielle Calderone Gray attempted to secure employment as a Registered Nurse (RN) unsuccessfully with her current employer, a family medicine clinic in the San Antonio, Texas area, where she had been employed as a medical assistant, by submitting a screen shot of a RN license to her employer. Danielle Calderone Gray was terminated after clinic management staff were unable to verify the licensure information she provided. Less than 2 months later, on or about September 6, 2018, Danielle Calderone Gray attempted to gain employment as a Registered Nurse (RN) unsuccessfully with a home health agency in the San Antonio, Texas area, by submitting an application which falsely indicated she had a license to practice professional registered nursing in the State of Texas. The home health agency staff grew suspicious when they were unable to verify the licensure information she provided. According to Board records, the information provided by Danielle Calderone Gray to the facilities did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. |

On or about March 5, 2018, through January 7, 2019, Jeanette Sue Allen was promoted to Patient Access-Registered Nurse with a local hospital in Seminole using falsified licensure information. During this time, Ms. Allen represented herself as a registered nurse (RN) while using the credentials of “RN” and title “Nurse” on the facility’s business and medical records. Ms. Allen fabricated a nursing license using the license number belonging to a nurse with a completely different first and last name.

On or about August 10, 2016, through October 12, 2018, Jessica Michael Porter obtained employment as a Health Service Administrator, a position that required registered nurse licensure, with a correctional facility in the Johnson County area. Jessica Michael Porter submitted a copy of a licensure record for an individual with the same first and last name from the State of Florida indicating she was a registered nurse. During this time, Jessica Michael Porter identified herself as a registered nurse (RN) while using the credentials “RN” on medical records and various other forms. Additionally, Jessica Michael Porter performed a variety of nursing duties, including patient care, tuberculosis testing, placing individuals on suicide watch, giving injections, and performing a sexual assault exam.
