Initial Licensure and Recognition Forms - Affidavit of Graduation - Vocational Nurse
The Affidavit of Graduation verifies that the applicant successfully completed all requirements for completion of graduation from an approved vocational nursing program as stated in Rule 214.9 of the Rules of the Texas Board of Nursing. The affidavit cannot be notarized prior to the completion or graduation date.
Affidavit Requirements
The affidavit must be completed by the Director of the Nursing Program only. This individual must be the registered nurse who is administratively responsible for the nursing program. The signature of other persons such as associate deans, program coordinators, or faculty members will not be accepted unless the Board has received official notification from the governing institution's administration that another nurse on the faculty has been given the authority to sign for the director, the length of time that the signature authority is valid, and a sample of the authorized person's signature.
By signing the document, the Program Director acknowledges that this is a legal document and understands that it is a violation of the 22 Texas Administrative Code, sections 217.12 (6)(H) and the Penal Code, sec. 37.10 to submit a false statement to a government agency.
Completed affidavit forms must be submitted by email to licensing@bon.texas.gov.

The Board has become aware of recent attempts to obtain nursing licensure in Texas through fraudulent representations concerning the completion of out-of-state nursing education programs. Please be aware that the Board will investigate any application that appears to be associated with fraudulent activity, and this may cause an additional delay in the approval of an application for licensure while the Board's investigation is ongoing. Further, an applicant's out-of-state nursing education program may be required to produce its program curriculum, policies, and procedures for inspection by the Board.
