Address Changes to be submitted through Nurse Portal
Board Rule 217.7 requires name and address changes to be submitted to the Board in writing. These changes may be submitted via fax, e-mail, or paper copy through the mail. Additionally, address and name changes may be submitted by using the Nurse Portal and uploading applicable supporting documents. Information about the Nurse Portal is available on the Board’s Nurse Portal webpage: https://www.bon.texas.gov/texasnurseportal/. Please be aware the Board will be amending its rules in the near future to require all name and address changes to be submitted through the Nurse Portal.
